Valor predictivo del roll-over test en pacientes con preemclampsia del servicio de gineco-obstetricia, Hospital Cayetano Heredia 2020

Objetivo: Determinar la efectividad del roll-over test como predictor de
preeclampsia en el servicio de ginecología y obstetricia del Hospital Cayetano
Heredia de Piura durante Enero a Diciembre 2020.
Métodos: El diseño de investigación del presente estudio es de tipo cohorte,
analítico, prospectivo, cuasi-experimental, se utilizó una muestra de 814
gestantes de entre 28 y 34 semanas.
Resultados: La edad de las gestantes en dónde se aprobaron la prueba de ROT
se establece que los pacientes menores de 35 años están en 73.9% y en
mayores de 35 años está presente en el 26.1%; en cuanto a la edad gestacional,
se presenta de 28 a 29 semanas en el 46% y en más de 30 semanas está
presente en el 54%; con respecto al antecedente de preeclampsia, está presente
en el 3.3% y el no presentar antecedente, está en el 96.7%; en el caso de la
sensibilidad presenta valores bajos con respecto a los antecedentes de
preeclampsia y enfermedades hipertensivas, sin embargo presenta mejores
indicadores para los valores de especificidad.
Conclusiones: La prueba de ROT es una evaluación efectiva para poder
predecir en gestantes que tenga elevación de presión con antecedente de
preeclampsia; además se evidencia que existe variación en la presión arterial
diastólica cuándo se cambia de posición decúbito lateral izquierdo a decúbito
supino produciendo diferenciales de presión que son sensibles a la prueba del
test ROT; se concluye que la sensibilidad del test es baja en el 17% pero
presenta una especificidad diagnóstica de 98%.
